The Urgent Care Plan for London (UCP) will enable clinicians across London to co-create and share urgent and advance care plans across clinical and organisational boundaries, integrating with electronic record keeping systems that are already being used by clinicians and the London Care Record.
The UCP will be hosted by NHS South West London ICB. There will be a dedicated team to support the transition process and the use of the UCP as an enabler for a wide range of clinical pathways. The UCP is planned to go live during summer 2022.
Arrangements have been made to make sure that the current care planning functionality continues beyond 1st April until the new Urgent Care plan goes live. This service is provided by NHS South West London. There will be no gap in the provision of a care planning service, and users are encouraged to continue creating care plans for patients.
