Shaping our Integrated Care Partnership priorities: A partnership discussion document was published in January 2023 and brings together months of working together to identify what the health and care needs are for our South West London population.
This discussion document describes how we have assessed the needs of our population and sets out our thoughts on the priorities for the Integrated Care Partnership.
It is the first stage in the development of our South West London Integrated Care Strategy, which the South West London Integrated Care Partnership (ICP) is producing to help set the strategic direction for health and care services across our six boroughs: Croydon, Merton, Kingston, Richmond, Sutton and Wandsworth.

We invited local people and our ICS partners to feedback on this document and discussed the comments and insights we received at our ICP meeting in April 2023.
Your feedback really makes a difference and will help to shape our plans for the future. The full integrated care strategy will be developed based on your feedback and published in summer 2023.
Our Integrated Care System is an opportunity to integrate health and care services, bring together partners in local government, the NHS and the voluntary and community sector and address deeply rooted health inequalities.
We recognise the importance of each of our six boroughs and the positive impact that local partnerships can have in their local communities. We want to ensure that the work we are doing across the whole of South West London has value and is relevant.
This discussion document is the product of conversations with health and care partners across South West London, our statutory organisations like NHS and Local Authorities, and our voluntary sector partners, Healthwatch colleagues, and our local communities.
All our partners share a determination to improve the health and wellbeing of our residents. This discussion document is the foundation for our collective action.
