NHS South West London’s primary care team in Kingston and Richmond is working towards the establishment of a Personalised Care approach for primary care. This will see the further development of the relationship between people, health professionals, and the wider system, with patients asked to consider ‘what matters to me?’ rather than ‘what’s the matter with me?’.
Many practices already work in this way, with residents being able to access self-care support through local community organisations. Local people can also see a range of different professionals, including social prescribers and health coaches who offer people broader support with practical, social and emotional needs that affect their health and wellbeing. An upskilling programme is now being offered to all practices which will refresh and strengthen this approach.
With training for our primary care colleagues starting next month, we look forward to local people feeling the benefits of the personalisation approach even more as we invite them to consider what matters to them and how we can improve their lives as a whole.”
